Extension for and interactive online pdf form for Joomla 1.5 that will email to either efax.com or interfax.com and print out on a fax machine exactly like the form and dimensions must be 8.5” by 14”.

Will substitute the name for “AMERICAN GENERAL” on the form for other firm which I will reveal later. Please if you can’t guarantee or have never done this before then don’t bid. Pros only needed here.

I do have a private party ready to do this but I want the prospective market value bid on this to make a valued decision.
